All volunteers must complete the Volunteer Application Form and Volunteer Code Form. In addition, if you are driving for a field trip the Volunteer Driving Form must be completed. On the second page of the Volunteer Driving Form there are instructions for completing a Driver's Abstract request which can be faxed to the school 604-903-3431. The Driver's abstract needs to be done every three years. All other forms need to be done every school year.
