Follow these steps, print the appropriate pages and return them to your coach. You are ineligible to compete until your coach has these materials. There are 4 steps in total, this process should take approximately 10 minutes to complete. There should be 4 items that you return to the coach:
1. student code of conduct
2. student awareness of risk
3. Informed consent (only print the last page)
4. Medical form
